My guess would be cost. Doesn't seem like it would cost extra to do, but from what I've read fiber installations can take anywhere from a few hours to all day and then some.... Multiply that by the number of customers and it begins to be a big deal as far as man-hours....

Short and quick answer for Bellsouth?? stop at the pole and use the existing copper wires (not quite as good as fiber, but certainly capable of 100 megabit connections)....
