I've also heard of this happening on game systems too. It can happen if the disc is cracked a little, in the middle for example. Some CD cases have pretty hard plastic, you almost have to yank it out. When you put the disc in the drive, the drive spins up, and when it reaches it's top speed, BAM!! There goes the disc.
